GreenShield will now take charge of all aspects of claims administration.
In case of a health issue, please follow these steps:
<ol>
<li>First, visit Maple&rsquo;s telemedicine services at <a href="https://getmaple.ca/greenshield" target="_blank" rel="noopener">getmaple.ca/greenshield</a>.</li>
<li>Consult a pharmacist or make an appointment with a physician at a private clinic or CLSC.</li>
<li>In case of severe symptoms, you should seek treatment at an emergency room of the nearest hospital.</li>
</ol>
If your health condition requires you to go to the hospital, here is the procedure to follow before going to the emergency room:
<ol>
<li>Call CanAssistance at 1 833 488-0277 to obtain assistance.</li>
<li>At the time of your visit, you must provide your GreenShield insurance card.</li>
<li>You must complete all documents requested by the hospital.</li>
</ol>
In the instance where you are required to pay for services received at the hospital, you must submit your claim directly to GreenShield using one of these methods:

Your Provincial Replacement Plan Summary
GSC pays up to 100% of the Provincial Health Plan's fixed schedule for the following covered services:
<ul>
<li>Physician's services in the home, the physician's office, the hospital or institution</li>
<li>One annual health examination</li>
<li>Services of specialists certified by the Royal College of Physicians and Surgeons of Canada</li>
<li>Diagnosis and treatment of illness and injury</li>
<li>Treatment of fractures and dislocations</li>
<li>Surgery</li>
<li>Administration of anaesthetics</li>
<li>X-rays for diagnostic and treatment purposes</li>
<li>Obstetrical care, including prenatal and postnatal care</li>
</ul>
Hospital &ndash; Inpatient Basis (Services included in the ward rate)
<ul>
<li>The usual and customary charges: standard ward accommodation and meals (room and board) - (up to 3 times the resident rate)</li>
<li>Private or semi-private accommodation when necessary and certified in writing by the attending physician</li>
<li>Necessary nursing services, when provided by the hospital</li>
<li>Laboratory and diagnostic X-rays procedures</li>
<li>Drugs prescribed by a physician (except when the hospital visit is solely for the administration of drugs)</li>
<li>Use of operating and delivery rooms, anesthetic and surgical supplies</li>
<li>Use of respiratory equipment</li>
<li>Use of radiotherapy facilities</li>
<li>Services rendered by any person paid by the hospital</li>
<li>Use of home renal dialysis and of home hyperalimentation equipment, supplies and medications where available in a hospital and prescribed by a staff physician of that hospital</li>
<li>Occupational therapy, speech therapy and physiotherapy in approved Canadian hospitals when prescribed by a physician as a medically necessary course of treatment</li>

Prescription Drugs
<ul>
<li>Co-pay: $5 plus 10% of remaining allowed cost, per prescription or refill</li>
<li>Mandatory Generic limited to a 30-day supply</li>
<li>Maximum: $15,000 per benefit year (Subject to the overall maximum)</li>
</ul>
Emergency Transportation, Private Duty Nursing in the home, Accidental Dental and Travel: No Co-pay
All Other Health Benefits: 20% co-pay
Private Duty Nursing
<ul>
<li>$200 per day up to 60 days per illness or injury per benefit year</li>
<li>$200 per day up to 30 days per illness or injury per benefit year for in home convalescent care</li>
</ul>
Paramedical Practitioners
<ul>
<li>Chiropractor, Chiropodist/Podiatrist, Osteopath, Physiotherapist, Psychologist, Acupuncturist, Dietitian, Occupational Therapist</li>
<li>$500 every benefit year per type of practitioner up to $1,000 for all practitioners combined</li>
</ul>
Eye Examination: Once per benefit year
Travel:
<ul>
<li>120 days per trip for training purposes (except in the U.S.)</li>
<li>14 days per trip for any other purpose (including the U.S.)</li>
