IMPORTANT:

As of June 1, 2023, GreenShield in partnership with CanAssistance, will provide coverage for your Provincial replacement plan.

CanAssistance offers a telephone assistance service 24/7. The phone number to call regarding your Provincial replacement plan is 1 833 488-0277.

GreenShield will now take charge of all aspects of claims administration.

In case of a health issue, please follow these steps:

  1. First, visit Maple’s telemedicine services at getmaple.ca/greenshield.
  2. Consult a pharmacist or make an appointment with a physician at a private clinic or CLSC.
  3. In case of severe symptoms, you should seek treatment at an emergency room of the nearest hospital.

If your health condition requires you to go to the hospital, here is the procedure to follow before going to the emergency room:

  1. Call CanAssistance at 1 833 488-0277 to obtain assistance.
  2. At the time of your visit, you must provide your GreenShield insurance card.
  3. You must complete all documents requested by the hospital.

In the instance where you are required to pay for services received at the hospital, you must submit your claim directly to GreenShield using one of these methods:

BENEFITS SUMMARY

Please note that starting June 1, 2023, claims must be submitted directly to GreenShield for the services usually covered by the Régie de l'assurance maladie du Québec (RAMQ).

Your Provincial Replacement Plan Summary

GSC pays up to 100% of the Provincial Health Plan's fixed schedule for the following covered services:

  • Physician's services in the home, the physician's office, the hospital or institution
  • One annual health examination
  • Services of specialists certified by the Royal College of Physicians and Surgeons of Canada
  • Diagnosis and treatment of illness and injury
  • Treatment of fractures and dislocations
  • Surgery
  • Administration of anaesthetics
  • X-rays for diagnostic and treatment purposes
  • Obstetrical care, including prenatal and postnatal care

Hospital – Inpatient Basis (Services included in the ward rate)

  • The usual and customary charges: standard ward accommodation and meals (room and board) - (up to 3 times the resident rate)
  • Private or semi-private accommodation when necessary and certified in writing by the attending physician
  • Necessary nursing services, when provided by the hospital
  • Laboratory and diagnostic X-rays procedures
  • Drugs prescribed by a physician (except when the hospital visit is solely for the administration of drugs)
  • Use of operating and delivery rooms, anesthetic and surgical supplies
  • Use of respiratory equipment
  • Use of radiotherapy facilities
  • Services rendered by any person paid by the hospital
  • Use of home renal dialysis and of home hyperalimentation equipment, supplies and medications where available in a hospital and prescribed by a staff physician of that hospital
  • Occupational therapy, speech therapy and physiotherapy in approved Canadian hospitals when prescribed by a physician as a medically necessary course of treatment

Your GSC Health Benefits Plan Summary

GSC

Calendar Year Deductible: No Deductible

Overall Health Maximum:  $750,000 per lifetime for all benefits combined including Travel Benefits

Prescription Drugs

  • Co-pay: $5 plus 10% of remaining allowed cost, per prescription or refill
  • Mandatory Generic limited to a 30-day supply
  • Maximum: $15,000 per benefit year (Subject to the overall maximum)

Emergency Transportation, Private Duty Nursing in the home, Accidental Dental and Travel: No Co-pay

All Other Health Benefits: 20% co-pay

Private Duty Nursing

  • $200 per day up to 60 days per illness or injury per benefit year
  • $200 per day up to 30 days per illness or injury per benefit year for in home convalescent care

Paramedical Practitioners

  • Chiropractor, Chiropodist/Podiatrist, Osteopath, Physiotherapist, Psychologist, Acupuncturist, Dietitian, Occupational Therapist
  • $500 every benefit year per type of practitioner up to $1,000 for all practitioners combined

Eye Examination: Once per benefit year

Travel:

  • 120 days per trip for training purposes (except in the U.S.)
  • 14 days per trip for any other purpose (including the U.S.)

Cigna Global Health Options provides an avenue to create a health plan tailored to dependents.  The primary policy holder must be 18 years of age.
