Use Change4Life&rsquo;s tools and resources to take small steps towards better health and earn points in the process. Then use points to bid on (and hopefully win!) cool rewards.
We've listed some of the great ways to earn points below.
<h3>Complete an online Health Risk Assessment (HRA)</h3>
Completing the HRA questionnaire allows the Change4Life portal to be customized to you. Responses to the HRA provide the foundation for what comes next &ndash; a personalized health &ldquo;report card&rdquo; with an overall health score, as well as a recommended action plan filled with targeted tips, tools and resources for adopting healthier habits. Haven't completed your HRA in the last six months? Take ten minutes to do it and earn 500 points.
<h3>Sign up to receive Stick2It medication reminders</h3>
GSC&rsquo;s Stick2It&reg; medication reminder service provides a helping hand in taking medication(s) as prescribed by a doctor and pharmacist. Set up Stick2It to receive regularly scheduled reminders to take medication(s) and/or refill prescriptions. Add a medication reminder and earn 100 points for every reminder you add.
<h3>Sign up to receive health reminders</h3>
Similar to Stick2It, stay on track with health-related activities by scheduling health reminders for a wide variety of health activities, such as tracking daily steps, physical activity, diet, blood pressure and many more. Earn 100 points for each reminder you add.
<h3>Learn about health topics</h3>
The health portal&rsquo;s educational modules help increase health knowledge. Learn about topics such as diabetes, stress, migraines, and many more. Complete "Learn &amp; Earn" modules and earn 25 points for each module.
<h3>Track your activities</h3>
Easily track your steps by syncing your wearables (such as Garmin and Wear OS by Google). Haven&rsquo;t synced up your wearable yet? That can get you up to 100 points for each day you reach 10,000 steps. Don&rsquo;t have a wearable? Update any of the trackers, including steps, fruit and veggie intake, and many more and earn 50 points per day.

<h3>Cardiovascular Health Coaching Program</h3>
What is the Cardiovascular Health Coaching Program?
This program offers support and coaching provided by pharmacists to help manage blood pressure and cholesterol, and to improve overall cardiovascular health.
Who is eligible for GSC's Cardiovascular Health Coaching program?
The program is available to plan members and eligible dependents (spouses and children) who have been diagnosed with both hypertension and elevated cholesterol and who:
<ul>
<li>Have GSC drug benefit coverage,</li>
<li>Are under 65 years of age, and</li>
<li>Are taking at least one drug to treat hypertension and one drug to treat high cholesterol.</li>
</ul>
Cardiovascular Health Coaching can help with the adoption of a healthier lifestyle.
This innovative program focuses on nutrition, exercise, smoking cessation, medication adherence, and personal health monitoring. Over four sessions &ndash; an initial assessment and three follow-up sessions &ndash; a pharmacist will:
<ul>
<li>Review and optimize medications to improve cholesterol and blood pressure</li>
<li>Establish goals that will have a positive effect on overall health</li>
<li>Assess &ldquo;heart age&rdquo; relative to actual age (to determine the risk of a heart attack and stroke)</li>
<li>At each follow up, the pharmacist will help assess progress and modify the plan as necessary</li>
</ul>
Is this covered by GSC plans?
Depending on the benefits plan design, a drug plan may fully reimburse the Cardiovascular Health Coaching program with no out-of-pocket costs to members. This is a value of $120 per year.
Ask a pharmacist about the program.
To find out whether a pharmacy is offering the service, simply ask a community pharmacist about GSC&rsquo;s Cardiovascular Health Coaching program.
If a pharmacist isn&rsquo;t currently offering the program, invite them to contact GSC to learn more about it and how to become a certified cardiovascular health coach.
Telephone coaching is also available.
Services are also available at home by contacting the North West Telepharmacy Solutions (NWTS)* dedicated phone line at 1.844.70.COACH (1.844.702.6224) or by emailing a request to participate to <a href="mailTo:cardiaccoaching@northwest.ca" target="_blank" rel="noopener">cardiaccoaching@northwest.ca</a>. A certified pharmacist health coach will respond and arrange a convenient time for the first appointment.

Cardiovascular disease is very common in Canada and is one of the leading causes of death. GSC has developed Cardiovascular Health Coaching to empower ownership and responsibility for health.
Those eligible for Cardiovascular Health Coaching are strongly encouraged to go ahead and participate. It&rsquo;s taking a big step towards improving health and reducing risk for heart attack and stroke. And a pharmacist will be there to help along the journey.&nbsp;
<h3>Smoking Cessation Program</h3>
What is the Smoking Cessation Program?
GSC&rsquo;s Smoking Cessation Program offers both smoking cessation drugs and counselling provided by a specially trained pharmacist. Try to quit with just the medications, just the counselling, or both. It&rsquo;s all about choice.
Want to stop smoking but are finding it a challenge?
Even with the best intentions, research indicates that most smokers make multiple quit attempts before successfully becoming a non-smoker. It is also well documented that the deal breaker &ndash; that makes all the difference between a successful and unsuccessful quit attempt &ndash; is a supportive environment.
The GSC Smoking Cessation Program is based on numerous studies that show improved success rates when programs offer choice and flexibility. Our program offers both smoking cessation drugs and/or support in the form of counselling provided by a specially trained pharmacist. It&rsquo;s all about choice.
Who is eligible for GSC&rsquo;s Smoking Cessation Program?
The program is available to all GSC plan members and dependents who want to quit smoking and are:
<ul>
<li>Covered for GSC drug benefits</li>
<li>18 or over for smoking cessation drugs and 16 or over for the counselling services</li>
<li>A resident of Alberta or Saskatchewan accessing smoking cessation drug therapy only. (Counselling is available under the provincial health plans)</li>
<li>Not an Ontario resident age 65 or older. (A similar program is available under the provincial health plan)</li>
</ul>
Ready to quit?
Ask a pharmacist about participating in the program. A pharmacist may also mention the program while providing another pharmacy service.
<ul>
<li>Complete a Readiness Assessment questionnaire.</li>
<li>Develop a quit smoking plan and establish a quit date at the first consultation meeting with a pharmacist.</li>
<li>Discuss whether to try a smoking cessation drug. The pharmacist can go over the options available under the program.</li>
<li>Attend seven follow-up sessions with the pharmacist over the next 12 months to discuss progress. Meet in person, over the phone, or via some other means of communication &ndash; whatever is arranged with the pharmacist is fine with us.</li>
</ul>

<h3>Pharmacist Deprescribing Program</h3>
About the desprescribing program
The Pharmacist Deprescribing Program helps members reduce or stop taking medication that is no longer helping or is doing harm, with the help and support of a pharmacist. Deprescribing (reducing or removing) from certain medications is especially helpful for those taking many medications.
The Pharmacist Deprescribing Program focuses on two drug categories:
<ul>
<li>Proton Pump Inhibitors (PPIs) &ndash; commonly used to treat stomach acid problems such as gastroesophageal reflux (GERD)</li>
<li>Benzodiazepine and Z-Drugs (BZRAs) &ndash; commonly used for sleep</li>
</ul>
A pharmacist will support and counsel through one-on-one sessions. The number of sessions depends on the drug:
<ul>
<li>PPIs &ndash; initial session and two follow ups</li>
<li>BZRAs &ndash; initial session and four follow-ups</li>
</ul>
Who qualifies for this program?
Take part in the Pharmacist Deprescribing Program if:
<ul>
<li>You are 18 years of age or older</li>
<li>A GSC plan member</li>
<li>Taking a PPI and/or a BZRA.</li>
<li>Taking a PPI or BRZA? Ask a pharmacist about the Pharmacist Deprescribing Program!</li>
</ul>

<h3>Dietitian Health Coaching Program</h3>
What is the Dietitian Health Coaching Program?
Making health conscious decisions when it comes to our eating habits isn&rsquo;t exactly the most intuitive. And while we&rsquo;re firm believers that the key to a healthy and happy diet is to enjoy everything in moderation (how can we say no to a slice of pizza?), we&rsquo;re well aware that lean protein and veggies surpass cheeseburgers and fries in the long run.
But because consuming a well-balanced diet is much easier said than done, we launched our Dietitian Health Coaching Program... because simply put, we can all use a little help (from a dietitian friend) once in a while.
About the Dietitian Health Coaching Program
The Dietitian Health Coaching Program, offered as part of GSC&rsquo;s Change4Life health management initiative, is a service provided by registered dietitians at select Loblaw stores across Canada. The program offers guidance and support that focuses on improving health by making better choices when it comes to food. &nbsp; The program consists of four sessions, delivered over four to eight months with a registered dietitian at a participating store location. There is also a virtual option available. &nbsp; Please note: benefits plans may not cover the Dietitian Health Coaching Program. Unsure? A Loblaw dietitian can confirm your coverage information. If it&rsquo;s not included &ndash; don&rsquo;t worry! GSC plan members can still benefit from GSC&rsquo;s Dietitian Health Coaching Program at a preferred rate (and just pay out of pocket).
